Integrated Care Clinician

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Integrated Care Clinician to work in a local primary care practice in our growing integrated care initiative.  

Integrated Behavioral Health Clinicians collaborate with medical providers and other members of the integrated team to assess patient needs for behavioral health support and treatment.  Through the provision of screening, assessment, brief intervention, and coordination of care, the Clinician works to address health behaviors and their contribution to chronic medical illnesses, life stressors and crises, stress related physical symptoms, and ineffective patterns of health care utilization.  In collaboration with the physicians in the practice, the Clinician provides direction and support to the team and provides education to staff and residents as appropriate about behavioral health issues.  

This is a full time position with principal responsibility to support the practice during clinic hours Monday through Friday and work alongside of, and fully integrate with, the practice staff.  The Clinician will attend practice meetings, implement screening programs, provide behavioral health assessment, crisis management, triage, targeted intervention, referral, care management and coordination.  The emerging system of coordinated care has numerous opportunities for learning and collaboration.  

Responsibilities

  • Engage in population-based education, prevention, and early intervention regarding behavioral health.
  • Provide training and consultation to the practice physicians and staff around the impact and management of patients' psychosocial and care coordination needs.
  • Utilize screening tools to identify patients in need of intervention.  
  • Provide assessment of patient behavioral health and care coordination needs, including diagnosis, to assist medical team in treatment of patient. 
  • Initiate collaboration with consulting psychiatric provider, integrated case manager, substance use clinicians, and other providers across the regional network when indicated.
  • Engage patients in brief, targeted interventions to improve patient functioning and well-being.  
  • Provide referrals to outside providers when longer-term or more comprehensive treatment is indicated, bridging care until referral is complete.
  • Provide education to individual patients or patient groups on behavioral health issues, skill techniques, and practices to engage patients actively and positively in their own treatment and positive health outcomes.
  • Adhere to all compliance procedures relevant to protected health information (PHI) and HIPAA regulations.  
  • Documents care provided in a shared electronic health record and meets compliance standards and practice standards.

Requirements

  • Master's degree in Psychology, Social Work, Mental Health Counseling or a related field is required
  • Eligibility for NH licensure as a LICSW, LCMHC, LMFT or MLADC is required; Active licensure is preferred
  • Valid driver's license and personal vehicle or access to a reasonable alternative is preferred.
  • Experience treating patients with a diverse clinical profile and clinical co-morbidities is preferred.
