Clinical Case Manager – Adults

 

Riverbend Community Mental Health, Inc., one of the largest providers of community-based mental health services in New Hampshire, is seeking a full-time Adult Clinical Case Manager to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a Riverbend Clinical Case Manager, your primary responsibilities will be working with clients to provide clinical consultation, case management and therapy services using a number of evidence-based models. You will work closely with treatment teams within Riverbend as well as outside agencies. This team serves primarily young adults and/or individuals with intellectual disabilities. This is a great opportunity to work on the front lines of community-based mental health services, and help our clients achieve optimal recovery and independence.

Essential Duties and Responsibilities:

  • Individual and group therapy both in the office and in the community
  • Case management and interagency coordination
  • Crisis management
  • Clinical assessments
  • Treatment planning
  • Clinical documentation in accordance with medical necessity guidelines and agency standards

 

Education and/or Experience Requirements:

  • Bachelor's degree in Psychology, Social Work, or related field required, Master's degree preferred.
  • Must have previously worked at least 1 year in a related human service or mental health position.
  • Valid driver’s license, personal vehicle, and auto insurance is required.
