Position Summary:

Here’s your opportunity to make a difference in the lives of local residents. We’re seeking a LICSW/Substance Use Disorder (SUD), Case Manager, to provide care management to patients using a “shared goal model”. You’ll facilitate the screening process and coordination of integrated care for our Behavioral Health patients and help provide advanced care management with the goal of improving patient safety and enhancing quality. Behavior care management may be provided in individual or group settings. This position will be accountable for the full patient care cycle. You must hold their LICSW, and or PhD/PsyD and be licensed or license eligible in the State of New Hampshire.

Essential Responsibilities:

  • Establish rapport with patients, assessing patient needs and work with them to achieve their goals.
  • Perform initial patient visit intake and assessment to include treatment plans and legal liaison services.
  • Work directly in treatment relationship with patients when needed.
  • Assist patients in defining goals, planning action, and gaining any skills or abilities reflecting strengths, interests, abilities, and needs.
  • Ensure service quality and participate in care coordination to ensure proper communication between clinical associates, patients, referral sources.
  • Help patient understand, accept, and follow medical recommendations.
  • Write psychosocial reports to be included as part of a patient’s Individual Plan of Service. Ensure report is included in Electronic Health Record.
  • Confer with Behavioral Health Director and other LICSW/LMHC to develop and implement the best treatment plan for each patient.
  • Promote healthy behaviors and coach’s patient toward successful self-management of behavior substance abuse disorder and management plan.
  • Manage care transitions between and among behavioral health, medical care providers and settings.
  • Report and document services in accordance with ACHS policies and procedures utilizing objective, measurable methods, and adhering to all regulatory time-frames and requirements.
  • Provide case management services to improve the skills and abilities of patients to function in the community.

Required Knowledge, Skills & Abilities:

Demonstrate evidence of essential leadership, communication, education, collaboration, and coaching skills.

  • Knowledge of alcohol and substance use disorder theory and treatment; a wide range of treatment modalities; therapeutic program services and crisis intervention techniques.
  • Possess knowledge of community-based programs for substance use disorder and other human services and operations and activities of an addiction treatment program.
  • Must be able to work independently.

Education & Experience Requirements:

  • One to three years’ experience in clinical or community health settings preferred.
  • Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred.
  • LICSW, and/or PsyD with at least three years of professional clinical experience in substance use disorder counseling or related field.

Certificates & Licenses Requirements:

  • Current BLS for Health Care Providers certification.
  • Current NH License or license eligible.
  • Must possess a valid driver’s license.
